Brooks Rehabilitation is seeking a Director of Rehabilitation to join their team at the brand new Brooks Aging Services Campus located in Bartram Springs in Jacksonville, FL!

The Brooks Aging Services campus includes a newly constructed 100 bed rehabilitation based, skilled nursing facility, 61-unit Assisted Living Facility and 2-twelve bed memory care centers, using a Greenhouse model of care.

Job Description: Responsible for all aspects of clinical management and staffing supervision for Program/service delivery within the Bartram Campus (SNF,ALF,Green House). Ensures development of employee specialty and expertise consistent with organization’s effort in establishing rehabilitation centers of excellence.

Job Requirements

Education, training, experience:
Required Skills: Physical Therapist or Occupational Therapist with a valid Florida License. 3 years experience in Skilled Nursing Management in a skilled nursing environment is required. Must have extensive knowledge of RUGs, Medicare standards of care and therapy delivery in a skilled nursing setting or facility.
Requires self-motivation, initiative and a high level of communication skills; promotes interdisciplinary collaboration toward patient care and outcomes; promotes care innovations through awareness of emerging literature and research; promotes programmatic approach to patient care and product line delivery; forms collegial relationships with all levels of management; fosters collegial relationships between management, treatment staff, nursing and medical staff across the continuum of care. Clinical skills consistent with designated professional licensure. Moderate level of computer/database skills as necessary to work in an electronic environment and analyze and interpret data for decision-making/problem solving/innovation.
